Background Check Authorization Form

To complete the Background Check Authorization Form, please follow these steps:

1. Click on the Download Background Check Authorization Form button. 
2. This will bring up the PDF page. Click the  icon to download the form on your computer.
3. Open the form in Adobe Acrobat Reader, complete the form fields, electronically sign and submit.
